Disability Pension ( Scheme )

ELIGIBILITY:

A. Age 18 years or above (Male/Female)

B. Domicile of Haryana and residing in Haryana for last 3 years at time of submission of application.

C. His/her close relative such as parents son, son’s, sons are not supporting him and his/her own annual income from all sources is not more than minimum wages of unskilled Labour as notified by Labour Department and circulated by department on year to year basis.

D. Disability 60% and above :-

  • Blindness
  • Low Vision
  • Leprosy Cured
  • Hearing Impairment
  • Locomotors Disability
  • Mental Retardation with I.Q. not exceeding 50.
  • Mental Illness

BENEFITS PROVIDED:

1. Rs. 1800/- per month (Rs. 2000/- w.e.f 1.11.2018 payable in Dec, 2018)

DOCUMENTS REQUIRED:

1. Disability certificate of 60% & above.

2. Birth Certificate

3. Aadhaar Card

4. Residential Proof : Any one of the following documents for the domicile of Haryana issued prior to 15 years to date of application will be accepted :-

  • Ration Card
  • Voter Card
  • Name of applicant in Voter List.
  • Pan Card
  • Driving License
  • Passport
  • Electricity bill/ Water bill
  • Documents of House and land
  • Copy of LIC Policy
  • Registered Rent Deed of House
  • Permanent resident certificate of Haryana

FEES FOR THE SERVICE:

Old Age Samman Allowance ( Scheme )

ELIGIBILITY:

1. Age 60 years and above.

2. Domicile and resident of Haryana.

3. Annual family income (Wife & Husband) less than or equal to Rs. 2 lakhs

4. School certificate issued by the school having DOB of the applicant is acceptable.

5. Exclusion: Notwithstanding the above, in case the person is receiving pension from any Government or Local/Statutory Body or any organization substantially financed by any Government or Local/Statutory Body will not be eligible to receive allowance under the Scheme.

BENEFITS PROVIDED:

1. Rs. 2000 /- per month

DOCUMENTS REQUIRED:

1.Age Proof: (Any one of the following)

  • A.
    Birth Certificate.
  • B.
    School Certificate of either 5th class or 8th class or 10th class as well as School leaving certificate issued by the Head Teacher of Primary School, Head Master/Head Mistress of Middle/High School alongwith copy of school record on basis which the certificate has been issued. The copy of such record attested by Head Teacher/Head Master/Head Mistress.
  • C.
    Driving License issued prior to 16.06.2016
  • D.
    Passport issued prior to 16.06.2016
  • E.
    Pan Card issued prior to 2005
  • F.
    Voter card issued or traceable prior to 2005
  • E.
    Name of applicant in voter’s list that shows photograph prior to 2005
  • H.
    Age proof of older child – If 40 years or more
  • Note:- If any of the above documents are not available with the applicant, in this case the applicant must go to the DSWO office and write a letter to the District Social Welfare Officer. So that the District Social Welfare Officer can refer the case to a team of two doctors constituted in the civil hospital of the district to assess the age of the applicant.

2. Residential Proof : Any one of the following documents for the domicile of Haryana issued prior to 15 years to date of application will be accepted :-

  • Ration Card
  • Voter Card
  • Name of applicant in Voter List.
  • Pan Card
  • Driving License
  • Passport
  • Electricity bill/ Water bill
  • Documents of House and land
  • Copy of LIC Policy
  • Registered Rent Deed of House
  • Permanent resident certificate of Haryana

4.Other Documents Saving Bank account detail of the applicant with a photocopy of the passbook.

Widow & Destitute Women Pension (Scheme)

ELIGIBILITY:

1. Women of Age more than 18 and permanent resident of Haryana for last 1 year at time of submission of application.

2. Annual income less than or equal to Rs. 2 Lakhs.

3. Any one of the three conditions are fulfilled:

  • 1. She is a widow; or
  • 2. She is destitute without husband, parents and son(s): or
  • 3. She is destitute due to desertion or physical/mental incapacity of,
    • (a) Husband in case of married women; or
    • (b) Parents in case of other women

4.Excluded category -

5. Notwithstanding the above any person getting pension from any govt or local statutory body will not be eligible to receive allowance under the scheme.

BENEFITS PROVIDED:

1. Rs. 2250 /- per month

DOCUMENTS REQUIRED:

1. F.I.R Copy of missing person’s report of the husband missing for the past 7 years.

2. Death Certificate of Husband ( in case of widow)

3. Destitute Certificate (in case of Destitute Women)

4. Aadhaar Card

5. Residential Proof : Any one of the following documents for the domicile of Haryana issued prior to 15 years to date of application will be accepted :-

  • a. Ration Card
  • b. Voter Card
  • c. Name of applicant in Voter’s List.
  • d. Pan Card
  • e. Driving License
  • f. Passport
  • g. Electricity bill/ Water bill
  • h. Documents of House and land`1
  • i. Copy of LIC Policy
  • j. Registered Rent Deed of House
  • k. Permanent resident certificate of Haryana

6. Other Documents Saving Bank account detail of the applicant with a photocopy of the passbook.

Note:- In case widow before marriage belongs to a other than Haryana the domicile of her husband having residence of Haryana for more than 15 years needs to be proved through any of the documents attached.
